We have the spray at our POE. Anyone who needs it, gets it. So far we haven't had to use it thankfully.

It's not that the spray is "more powerful" per se, its the dose thats contained in the spray bottle is larger. The Naloxone intranasal spray that the RCMP carry contains a 4mg dose. That's a lot of naloxone. A normal first dose is typically 0.8mg ... and 0.8mg is enough to bring the majority of OD patients around.

In terms of efficacy of drug administration routes, intranasal (spray) and intramuscular (injection) are almost equal. Both take roughly the same amount of time for the medication to take effect. The IN spray is great because it is easier to use and no one is going to get an accidental needle stick from a spray bottle.

The trouble with a larger dose is it can bring people up too quickly. The actual emergency for these cases is the fact that they're not breathing. Their blood oxygen levels drop and the their CO2 levels rise, this causes their brain to swell.Bringing them up fast without providing ventilations to correct their O2 and CO2 levels is why they can come up super combative. Its not because they're an asshole and they're mad you took their high away (though that can also be true), they're combative because their brain is swollen and they have no idea what's going on.

Large doses can also cause addicts to go into withdrawl, which is a lovely present to deliver to the hospital. With a small dose, they can walk in and sit in a chair in the Emergency Department and be monitored for a few hours. If they're in withdrawal, now they're taking up a bed, rolling around, vomiting, possibly seizing, etc ... nurses love that shit ...
